However, a standard GP referral letter should suffice, and that non completion of a Portal entry for that private provider should not mean refusal of a referral.
My Planned Care
The My Planned Care website supports people waiting for a hospital appointment, operation or treatment, and give advice and support while they wait. This includes access to average waiting times at the hospital and other useful advice and local services.
This site is updated weekly and can be viewed by anyone, which means you, your family or carer as well as your NHS team can all see the latest information.
Your hospital team will be in touch with you as soon as they can. If you are looking for an update, please check this website before contacting your hospital or GP.
Urgent Referrals
If your doctor or nurse want you to be seen urgently by a specialist ‘to rule out the possibility of cancer’
- you should be given a ‘2 Week Wait Form’ in your appointment which explains what happens next.
- we will make a ‘2 Week Wait’ electronic referral direct the to the hospital within 1 working day.
- the hospital should contact you within a few days.

Routine Referrals
If your Clinician told you they were referring you for a Routine referral:
Our Secretaries will send a Text, Email or Letter to you within 7 working days.
It will tell you where you have been referred, which speciality, and a Telephone Number to contact their Referrals Booking Team.
You may also give you a Unique Booking Reference Number (UBRN) to quote.
Most referrals are sent for processing to the University Hospitals Sussex Outpatient Booking Hub. They will contact you.

Please check with them if an appointment has been booked for you – but please note the Significant delays at the moment.
If you asked your Clinician for a referred to a different hospital (such as Worthing), you may need to ring that hospital direct and ask for their Referrals Booking Team.

Here are some of the ways you can reduce your risk of falling:

- Strength & Balance – Ageing Well
- A chiropodist is trained to look after people’s feet and advise you on proper foot care. Ask at the GP practice to see an NHS chiropodist (also known as a podiatrist), but be aware that there may be restrictions on who can be referred. To find a private chiropodist or podiatrist, contact the College of Podiatry 020 7234 8620
- If you have diabetes, you should have your feet screened as part of your annual diabetes review to look for, and monitor, any changes
- Sight tests – Sight and hearing can also affect your likelihood of falls. You can find your local NHS optician services, some also offer home visits.
- Hearing – You can test your hearing online or at local audiologist services. The RNID have a 3 minute hearing test. It is free and online.
- Medication Reviews – It’s also important to have a regular review of any medications you are on as certain medicines can sometimes cause dizziness or drowsiness and, along with other factors, contribute to the risk of falling over.
- If you are worried about your medications and/or your risk of falling, please contact your pharmacist or GP Practice to discuss before taking any steps to reduce them.
